A photo ID is required to access some records and records requests may incur copying and research fees. There is a PDF Records Request form provided online. Requests should describe the records requested in as much detail as possible, including relevant names, locations, dates, departments, etc. Requests may be mailed, emailed, or delivered in person. The Records Division is open 8:30am to 4:30pm Monday through Friday.
